Creative Connections Waiver

 

“ I am agreeing to have my child(ren)participate in "Creative Connections" Classes, to take place at Ysabel Barnett Elementary School, I HEREBY WAIVE, RELEASE AND DISCHARGE ANY AND ALL CLAIMS FOR DAMAGES FOR DEATH, PERSONAL INJURY OR PROPERTY DAMAGE WHICH I MAY SUSTAIN, OR WHICH HEREAFTER ACCRUE TO ME, AGAINST HANDS ON AS A RESULT OF MY PARTICIPATION IN THESE ACTIVITIES.  This waiver is intended to discharge Creative Connections, officers, employees, contracted teachers and authorized volunteers from and against any and all liability arising out of or connected in any way with me or my child's participation in these activities, except where caused by the active negligence, sole negligence or willful misconduct of the aforementioned. I understand and agree that this waiver, release and assumption of risk is to be binding on my heirs and assigns.  I am the parent or legal guardian of this participant. I understand the legal consequences of this document and I am signing it freely. I understand that I am responsible for the obligations and acts of the Participant as described in this document. I agree to be bound by the terms of this document. I have provided accompanying written parental/guardian consent below to participate in this activity."

 

I hereby give Creative Connections the right and permission with respect to the photographs(names will NEVER be included) that he/she has taken of my child or myself (while participating in Creative Connections Classes):

To use, re-use, publish and re-publish the same in whole or in part, separately or in conjunction with other photographs, for Creative Connections website, advertisements, Instagram, Facebook and TikTok pages.

I hereby release and discharge Creative Connections from all and any claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy.
